Collapse of Structures from Construction Procedures

-         An investigation and analysis of the collapse of a Truss building showed the building was not erected safely during construction and the building trusses collapsed prior to decking and injured a worker

-         A scaffold was improperly constructed and allowed the platform to collapse and injured a worker.

Collapse of Signs from Wind and Hurricanes

-         A large double sided sign’s panels broke apart during high winds and killed bystander. Inspection and Analysis showed the sign was of a defective design and not manufactured to standards for the wind zone for construction.

-         A large double sided sign broke off from the steel column and destroyed a manufacturing facility below the sign. Analysis showed the sign was of a defective manufacture resulting in the collapse of the sign.

Collapse of Structures from Wind and Water Load with Time Evaluation

-         Prior to Hurricane Ivan coming ashore the forward winds collapsed a shoreline cottage before the flood’s elevation washed away the cottage structure. Analysis of the age of the house and construction standards in place at the time of construction failed to meet wind loads recorded during the Ivan wind event.

Miscellaneous Wind damage

-         Roof damage repair post Hurricane Ivan was repaired however repair was defective and required testing and further repair to prevent internal leaks to the structure.

-         A metal building in a Hurricane experienced some permanent damage to the roof panel opening the overlapping seams and allowing water leaks into the building.
